• Get a list of certain disclosures we have made.

• File a complaint if you believe your privacy rights have been violated.

How We May Use and Disclose PHI

• Treatment: We may use and disclose health information to support treatment and care coordination by healthcare professionals involved in care.
• Payment: We may use and disclose health information to support payment-related activities where applicable and permitted.
• Healthcare operations: We may use and disclose health information for quality improvement, care coordination support, training, auditing, business planning, customer service, platform administration, and related operations.
• Business associates and service providers: We may disclose health information to organizations performing services on our behalf that are required to protect it under applicable law and contract.
• As required by law: We may use or disclose health information when required or permitted by federal, state, or local law.
• Public health and safety: We may disclose health information for public health, reporting, oversight, or safety activities as permitted by law.

Where applicable, we are required by law to maintain the privacy and security of PHI, provide this Notice, follow the Notice currently in effect, and notify affected individuals following a breach of unsecured PHI when required by law.

We may change the terms of this Notice. Changes may apply to PHI we maintain. The current Notice will be made available through the website, the app where applicable, and upon request.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below. You may also file a complaint with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
