• Treatment: We may use and disclose your health information to support treatment and care coordination by healthcare professionals involved in your care.
• Payment: We may use and disclose your health information to obtain payment for healthcare services or related operations, where applicable.
• Healthcare Operations: We may use and disclose your health information for quality improvement, care coordination support, training, auditing, business planning, customer service, platform administration, and related healthcare operations.

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below. You may also file a complaint with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
